How Do We Identify the Causes of Sleep Apnea?

Sleep Endoscopy: How Do We Identify the Causes of Sleep Apnea?

Obstructive sleep apnea may be caused by obstruction at different points of the upper airway, such as the soft palate, the base of the tongue, or the laryngeal region.
To treat sleep apnea effectively, it is important to know exactly where the obstruction occurs during sleep.

What is sleep endoscopy?
Sleep endoscopy is a specialized examination that allows us to observe the upper airway while the patient is under controlled drug-induced sedation, which simulates certain characteristics of natural sleep. Using a thin, flexible endoscope, we examine the passage of air from the nose to the lower parts of the pharynx and, depending on visibility, the laryngeal region. This allows us to identify the points where the airway narrows or closes.

How is the examination performed?
The examination is performed in a suitably equipped medical facility, with continuous monitoring of the patient and under the supervision of a specialized anesthesiologist. It usually lasts a few minutes. After its completion, the patient remains under observation until sufficiently recovered and the criteria for safe discharge have been met.

Why is it important?
Sleep endoscopy provides valuable information about the mechanism of obstruction and contributes to planning the appropriate treatment for each patient. Identifying possible obstruction at the base of the tongue is particularly important. In selected patients, assessing the airway's response to advancement of the lower jaw can help determine whether a specialized oral appliance that holds the lower jaw in a forward position during sleep is an appropriate treatment option.

Individualized Treatment of Sleep Apnea
Every patient is different. Sleep endoscopy, in combination with a sleep study, clinical examination, and medical history, helps us select the treatment approach that best meets each patient's needs. Our goal is to identify the cause of the obstruction and develop an evidence-based, individualized treatment plan for the effective management of sleep apnea.
